Pulmonary abscess, also known as lung abscess, is a category of disease within the field of Internal Medicine in traditional Chinese medicine, falling under the category of lung-related diseases. It is defined as a condition where the lung is affected by external pathogens or due to internal injuries and fatigue, leading to weakened lung Qi, accumulation of heat-toxin, Qi stagnation and blood stasis, resulting in the loss of lung's normal purification function, and thus presenting with clinical symptoms such as cough, chest pain, fever, and expectoration of foul-smelling sputum containing pus.
The pathogenesis of pulmonary abscess primarily involves the accumulation of heat-toxin in the lung and Qi stagnation with blood stasis. Common triggering factors include external wind-heat invasion, irregular diet, and internal injury due to overexertion. During the differential diagnosis, attention should be paid to distinguishing between cold and heat, deficiency and excess. In the early stage, symptoms such as aversion to cold, fever, cough, chest pain, and small amounts of white sputum are prevalent, often indicating wind-heat invasion of the lung, and treatment should focus on dispersing wind-heat and clearing lung phlegm. In the mature abscess stage, the chest pain intensifies, accompanied by expectoration of turbid sputum with a yellow-green color, a sensation of a foul smell in the throat, indicating heat-toxin congestion in the lung. Treatment should then aim to clear the lung and detoxify, resolve blood stasis, and dissipate the abscess. In clinical practice, the treatment of pulmonary abscess in traditional Chinese medicine emphasizes holistic adjustment, reinforcing the healthy Qi and eliminating pathogenic factors, nourishing lung Qi to restore the normal physiological function of the lung. Traditional Chinese medicine holds that the treatment of pulmonary abscess should follow the principles of clearing heat and detoxifying, activating blood circulation and resolving stasis, and nourishing yin and moistening the lung.
