Traditional Chinese Medicine Definition
In traditional Chinese medicine, spermatorrhea, also known as slippery semen, falls under the category of kidney-related diseases. It refers to the condition where an adult male experiences the spontaneous ejaculation of seminal fluid without engaging in sexual intercourse. The classical text "Internal Medicine in Traditional Chinese Medicine" records: "Spermatorrhea is the spontaneous emission of seminal fluid without sexual intercourse." This condition is often attributed to deficiencies in kidney essence, internal accumulation of damp-heat, or excessive heart fire.
The clinical辨证 approach to spermatorrhea is as follows: Firstly, the core pathogenesis of spermatorrhea is the deficiency of kidney essence, leading to its inability to be retained. Common triggers include emotional disturbances, irregular diet, and excessive sexual activity. During the process of differentiation, the following key points should be noted:
1. Exuberant King and Minister Fire Syndrome: Patients experience insomnia, frequent dreaming with spermatorrhea, restlessness, and dizziness. This syndrome is often caused by excessive heart fire, which disturbs the seminal chamber.
2. Downward Flow of Damp-Heat Syndrome: Patients experience frequent spermatorrhea, hot and turbid urine, and bitter taste in the mouth. This syndrome is often due to internal accumulation of damp-heat, which descends to the kidneys.
Overall treatment direction: In clinical practice, the treatment of spermatorrhea is based on the principles of nourishing the kidneys and consolidating essence, and clearing heat and promoting diuresis. For the Exuberant King and Minister Fire Syndrome, the approach should be to clear the heart and drain the liver, and to facilitate communication between the heart and kidneys; for the Downward Flow of Damp-Heat Syndrome, the approach should be to clear heat and promote diuresis, and to calm the mind. Traditional Chinese medicine believes that adjusting lifestyle, dietary adjustments, and emotional management can help improve the condition.

Physician's Clinical Insights
For patients with spermatorrhea, it is essential to pay attention to the timing, frequency, and accompanying symptoms of spermatorrhea to differentiate between deficiency and excess, and to assess the degree of kidney essence deficiency.
