Traditional Chinese Medicine Definition

Malaria, also known as malaria syndrome, falls under the category of diseases related to the liver and gallbladder in traditional Chinese medicine (TCM). According to the records in "Internal Medicine of Traditional Chinese Medicine," malaria is an infectious disease caused by malaria parasites, primarily transmitted through mosquito bites. It is characterized by alternating chills and fevers, with intermittent clinical manifestations such as intermittent chills, high fever, sweating, and so on. The pathogenesis of malaria is mainly related to disharmony between营(guardian Qi) and卫(defensive Qi), as well as the obstruction of Qi and blood circulation.

In the clinical practice of TCM, the core pathogenesis of malaria is the disharmony between营(guardian Qi) and卫(defensive Qi), and the obstruction of Qi and blood circulation. Common triggers include exposure to malaria pathogens, irregular diet, excessive fatigue, and more. During the differential diagnosis, attention should be paid to distinguishing between cold, heat, deficiency, and excess, such as in the case of regular malaria, which presents with chills followed by intense fever, with intermittent发作, starting with yawns and fatigue, followed by chills, and then a feeling of heat both internally and externally after the chills subside; in the case of warm malaria, there is more heat than cold, with sweating that is not smooth, thirst with a desire for drinks, and headache. The overall direction of treatment suggests expelling pathogenic factors, harmonizing the exterior and interior, and clearing heat and resolving the exterior while harmonizing and expelling the pathogenic factors. In TCM clinical practice, the treatment of malaria focuses on harmonizing营(guardian Qi) and卫(defensive Qi) to restore the normal circulation of Qi and blood, with the aim of expelling the pathogenic factors and restoring health. Traditional Chinese medicine believes that through differential diagnosis and treatment, combined with herbal medicine, acupuncture, and other methods, it is possible to effectively alleviate the symptoms of malaria and improve the quality of life for patients.

Common Syndrome Types and Key Points of Differentiation

正疟

Main Manifestations:寒战壮热,休作有时,先呵欠乏力,继则寒战,寒罢则内外皆热

Management Approach:祛邪截疟,和解表里

温疟

Main Manifestations:热多寒少,汗出不畅,口渴引饮,头痛

Management Approach:清热解表,和解祛邪

The above syndrome information is based on traditional Chinese medicine textbooks and clinical standards. Specific differentiation should be completed by a practicing traditional Chinese medicine physician in person.

Physician's Clinical Insights

For physicians, the regularity of the onset and remission of chills and fever during an attack of malaria, as well as the physical condition during the remission period, are crucial for differential diagnosis.
